HFKC Junior Soccer
Code of Conduct and Injuries
I confirm that I have read and understood, and I agree to abide by, the Parent’s Code of Conduct,
including a parent’s obligation to have a responsible adult present at soccer activities at all
times. I confirm that this responsible adult will be responsible for the care of my child in
the event that my child is injured during soccer activities. In the event that my child is injured,
and I have not arranged for the presence of a responsible adult, or the responsible adult is
not at the scene when the injury occurs, I agree that the Hong Kong Football Club (the "HKFC")
and its representatives may do whatever in their discretion is necessary to attend to such injury,
including calling Emergency Services, without the need first to contact the parent or look for
the responsible adult, and I agree to indemnify and hold harmless the HKFC and its representatives
against all liabilities and expenses incurred as a result and waive all rights and claims I may
have against the HKFC or its representatives in connection with, or arising out of, such attendance
and/or the calling of Emergency Services. I understand and acknowledge that the HKFC does not
carry insurance in relation to accidents or injuries to participants in soccer activities and
confirm that I will maintain valid, appropriate and comprehensive insurance in respect of these
risks.
Photographs and Videos
I understand and have no objection to the fact that the HKFC may take or receive photographs or videos
of participants in various soccer activities, including the activities forming part of the Junior
Soccer programme, and that these photographs and videos may be posted on the HKFC's website,
used in promotional material, or published or distributed in other ways without any consent,
acknowledgement or compensation being required or any liability of any kind attaching to the
HKFC or its representatives, or to the photographer or videographer (if independent of the HKFC).
Privacy
Your privacy is important to the HKFC. We may ask you to provide personal information in the course
of your involvement with the Junior Soccer programme, including via our website. The data that
we may ask you to supply includes name, date of birth, gender, telephone number, e-mail address,
residential address, HKFC membership number, and payment account details. From time to time,
we may use your name and e-mail address to provide you with details of other soccer or sports-related
events to be held at the HKFC or other venues (which may include events not organised by the
HKFC). We may disclose and transfer (whether in Hong Kong or abroad) your personal data to third-parties
to assist us with providing you with such information. These third-parties will only be permitted
to use your personal data for the purposes described above and not for their own purposes (including
direct marketing).
I accept and agree to be bound by the Hong Kong Football Club's Junior Soccer Code of Conduct for
Players, Coaches and Parents.
By ticking the "I Agree" box you agree and consent to the Hong Kong Football Club Junior Soccer terms
and conditions.
Real Madrid Foundation's Clinic (Hong Kong)/Dreams Foundation
1. I agree to allow my child to participate in the sport program and competition co-organized by the Dreams Foundation the " REAL MADRID FOUNDATION’S CLINICS (Hong Kong)", and I also re-affirm that my child's physical condition is able to cope with the aforesaid training and competitions. I understand that the Dreams Foundation assumes no responsibility for any accident or injury in training or during competitions. Participants may need to purchase their own casualties and protect property insurance.
2. No later withdrawals are accepted for the students have completed all the registration requirements and during the course; therefore, no refund will be given. For any physical condition of the student is not suitable to participate in activities, please contact our staff, we will handle case by case.
3. Privacy Policy
This Statement is made by REAL MADRID FOUNDATION’S CLINICS (Hong Kong)/ Dreams Foundation in accordance with the Personal Data (Privacy) Ordinance Cap. 486 (“Ordinance”) of the Hong Kong Special Administrative Region of The People’s Republic of China. The Statement is intended to inform you about our policies and practices with respect to the collection, use, retention, disclosure, transfer, security and access of personal data.
REAL MADRID FOUNDATION’S CLINICS (Hong Kong)/Dreams Foundation is committed to ensure that your privacy is protected. Should we ask you to provide certain information by which you can be identified when using this website, then you can be assured that it will only be used in accordance with this privacy policy.
We may collect the following information:
- Student name, Identification No. and parents name
- Contact information including mobile phone and email address
- Demographic information such as date of Birth, name of School and interests
- Other information such as health status or offers
We require the information to understand your needs and provide you with a better service, and in particular for the following reasons:
- For internal record keeping.
- To improve our products and services.
- To send promotional emails about new products, special offers or other information which we think you may find interesting using the email address which you have provided.
- To contact you for market research purposes. We may contact you by email, phone, fax or mail.
- To customize the class according to your interests.
- Registration members of Real Madrid Foundation/ Dreams Foundation and related uses
To ensure that your information is secure and to prevent unauthorized access or disclosure, we have put in place suitable procedures to safeguard and secure the information we collect online.
During the REAL MADRID FOUNDATION’S CLINICS (Hong Kong)/ Dreams Foundation training we will have photo shooting, Dreams Foundation reserves the right to use the images of the participants for publication and promotional purposes.
In case of any inconsistency or conflict between the English and versions of other languages, the English version shall prevail.
